Patient Forms

It is the patient’s responsibility  to contact their insurance to confirm coverage and need for referral.  

Please Note: if your policy requires a referral from your primary care physician, please secure this referral prior to making your appointment.

 

Insurance Information

We are preferred providers on most plans in the Seattle area but it is the patient’s responsibility to contact their insurance to confirm coverage and need for referral. For detailed information on referrals, go to our referrals page.

As a courtesy, we will bill your insurance plan for you, if you would please:

  • Bring your insurance card at the time of your visit
  • Contact your insurance company to confirm the following:
    • What is included in your benefits
    • What co-pays or deductibles apply
    • Are we a participant in your plan?
    • Do you need a new referral or updated referral from your primary care physician?

In the event a patient does not have an insurance card on file, or does not have required referrals or pre-authorizations on file, there will be a $100 fee to be paid prior to seeing the doctor. If tests or procedures are performed during the visit, additional charges may be due at the end of the visit. Once we receive appropriate insurance documentation, your insurance company will be billed and you will be promptly refunded after the insurance company remits payment.
